Survey Completion and Submission Deadlines
Required Action
All Space Survey results must be submitted through the online portal no later than 5 pm on August 25, 2009 for all departments. We can receive your Space Survey Certification Form and hard-copy of your Survey Summary results after August 25, but electronic results must be in before then. The Space Survey portal will be closed after the August 25 date.
After you have completed updating every room record for your Base Department, and before you submit the results electronically through the Space Survey portal, print out a hard-copy of your Survey Summary report. After you have your hard-copy, submit your results electronically through the Space Survey portal upon logging out. Attach your Survey Summary results to the Certification Form, have the Space Delegate and Space Official certify the results (i.e. sign the form), and route the completed form with attached hard-copy results to the Facilities Office Building, Room 120, Attention: Facilities Inventory.

I. Survey Summary Report
- Click on the Report button below the UNT brand
- Click on Survey Summary
- Print out a copy in landscape page orientation
II. Space Survey Certification Form
- Visit the Facilities Inventory website
- Find item number 9 under the heading “Steps in the Process” and choose the certification form under that step.
- Download the form, fill in the required information, sign it, and attach it to the hard-copy results. Then, route accordingly.
Steps to Complete
- Print out your Survey Summary report – see Data Source, Section I above.
- Log out of the Survey and click the button that says “Submit Survey Data” – this will lock your department’s survey and send the data to Facilities Inventory for processing.
- Print out the Space Survey certification form – see Data Source, Section II above.
- Have the Space Delegate for the Base Department certify the results (i.e. provide their signature on the form).
- Route the form, with Survey Summary attached, to the Space Official.
- Contact FIMS staff if you need assistance figuring out who your Space Official is
- The Space Official certifies the results.
- The completed form and Survey Summary are routed to the Office of Facilities Management and Construction.

Background Information
The online Space Survey portal will be initialized on August 3, 2009 – you can start logging in and making changes that day.
We have a very small staff to process survey data for over 14,000 rooms, verify the data, correct floor plans, if necessary, and post the necessary updates to our database, in addition to extracting the data to create the state-mandated report. Please start your surveys as soon as possible and submit as soon as possible, as well. We hope to avoid receiving all submissions for each group on the last day. We appreciate the time and effort spent in completing your survey and thank you in advance for your cooperation.
